What is the Swing Bed Program?
The Swing Bed Program provides care for patients who no longer require acute care hospitalization but require additional recover time prior to discharging home. Our dedicated staff will help you make a smooth transition from the hospital setting to your home or extended care facility.
The Swing Bed Program provides additional therapy after a hospital stay, including:
- rehabilitation after orthopedic or major surgery, or fractures,
- stroke care rehabilitation,
- continued wound care,
- strength and mobility restoration,
- continuous intravenous therapy.
Our goal is for you to return home with the same level of independence you had prior to the hospitalization.

- Pharmacy Services will strive to provide medication therapy.
- Nursing Services will work closely with all members of your care team. Nursing care is provided to all Swing Bed patients 24/7.
- Respiratory Therapy will work closely with physicians and nursing staff to determine a plan of care that best meets your goals.
- Rehabilitation Services helps you regain functional independence through exercises, patient/caregiver training and hands-on therapy.
- Registered Dietitian services may provide nutrition support and education, as needed.
- Case Manager will assist the healthcare team with care coordination, referrals, and social service support.

program eLIGIBILITY
Your Swing Bed Program eligibility is reviewed daily. To continue in the program, you must be participating and making progress, in additional to other criteria listed.
- You have an acute care inpatient hospitalization for a minimum of 3 consecutive midnights within a 30-day period. (Not including observation days)
- You are medically stable.
- You have a physician authorizing daily skilled care needs directly related to the acute care hospitalization.
- You must be willing to participate in the daily skilled care; five (5) days a week for rehabilitation.
Please note: Insurance plans vary regarding eligibility. We recommend you contact your insurance company for information on specific coverage.
Medicare part A will cover up to 20 days in full and up to an additional 80 days with a patient co-pay, as long as criteria is met. Medicare supplemental insurance may pay the deductible as long as Medicare coverage continues. Your coverage will depend on how long you meet criteria. The average length of stay for a Swing Bed is one to two weeks.
